※講演会ご参加の方で年会費も一緒に振込みの方は有りを選択して下さい。
※「有り」の方の領収書の宛名は、法人名＋個人名で用意いたします。
Affiliation + your name will be written as the addressee of a receipt.
※領収書の宛名の変更をご希望の方は、備考欄にご希望の宛名をご記入ください。
If the addressee should be changed, please type the addressee you hope within "Remarks".
※「無し」の方は総会以後、事務局より別途ご連絡いたします。
